Online referee logins have been disabled for the season

For the remainder of the spring/summer season, in order to receive payment
for a 2009 League game in which you officiated, you must now:

• Fill out the reverse side of your Password 'postcard' with your full
  name and mailing address.
• Enter the game field, game date and time and the names of each
  of the participating teams.
• Place the completed Password 'postcard' in an envelope and
  mail it to:

the requested information, so to expedite payment, please make
